Título: Gingival Squamous Cell Carcinoma Mimicking a Reactive Lesion

Resumo: A 68-year-old woman was referred for evaluation of a painless, reddish, lobulated nodule located in the upper anterior gingiva, with a duration of seven months. Her medical history was unremarkable, and she denied both tobacco and alcohol use. The adjacent teeth (maxillary canine and lateral incisor) were vital, with only minimal bacterial biofilm observed in the gingival sulci. The initial clinical diagnosis was pyogenic granuloma, and an incisional biopsy was performed. Histopathological analysis revealed infiltrative islands and cords of atypical epithelial cells with keratin pearl formation, nuclear pleomorphism, and invasion into the underlying connective tissue-features diagnostic of squamous cell carcinoma (SCC). The patient is currently undergoing oncologic treatment. Although uncommon, gingival SCC may clinically mimic reactive lesions, posing a diagnostic challenge. This case emphasizes the critical role of histopathological evaluation in the assessment of persistent gingival lesions with benign clinical appearance.
