Resumo: The incidence of syphilis has risen significantly in Brazil and the Americas in recent years, posing a major public health challenge, particularly among young adults. Oral manifestations of syphilis can occur at any stage of the disease, presenting as hard chancres, mucous patches, or gummatous lesions. However, atypical oral presentations can complicate diagnosis. This study describes three atypical cases of oral syphilis with diverse clinical features: 1) ulcerated, hemorrhagic lesions with desquamation affecting the oropharynx, tongue, lips, buccal mucosa, and gingiva, resembling stomatitis areata migrans; 2) a solitary lesion on the lower lip, associated with a history of trauma and mimicking squamous cell carcinoma; and 3) a plaque-like lesion on the tongue. Following comprehensive clinical evaluation and history-taking, serological testing for syphilis (VDRL and FTA-ABS) confirmed the diagnosis in all cases. Patients were referred to an infectious disease specialist for treatment and monitored until complete resolution of the lesions.
