Resumo: A 64-year-old man presented to a hospital dental clinic with a 48-hour history of a buccal nodule and oral bleeding. His medical history included controlled hypertension, diabetes, and a prior myocardial infarction. Intraoral examination revealed multiple vascular lesions (the largest measuring 1.5 x 1.0 cm on the buccal mucosa) and spontaneous gingival bleeding. Suspecting a blood dyscrasia, complementary tests confirmed severe thrombocytopenia (5,000/mm³). The patient was urgently hospitalized in the ICU, receiving dexamethasone pulse therapy, platelet transfusion, and antiparasitic prophylaxis. Oral lesions improved rapidly, but persistent thrombocytopenia necessitated methylprednisolone, prednisone, and immunoglobulin therapy. Platelet counts subsequently rose to 140,000/mm³, prompting discharge. Further investigation ruled out secondary causes, confirming idiopathic thrombocytopenic purpura (ITP). Medical and dental follow-up over nine months revealed no recurrence. Interdisciplinary management enabled swift disease control and prevention of severe complications, underscoring the pivotal role of oral manifestations in diagnosing systemic conditions.
