Resumo: A 43-year-old male patient presented with an asymptomatic swelling evolving over five years. The lesion was firm on palpation, sessile, on the left side of the palate. Its surface showed an area of ulceration and telangiectasia. CT imaging revealed a predominantly hypodense lesion with ill-defined borders, invading the nasal cavity and left maxillary sinus. Histopathological analysis revealed a hybrid neoplasm with an infiltrative growth pattern, consisting at times of cells with pleomorphic nuclei arranged in cords and islands, as well as duct-like structures containing PAS-positive material, and at other times of solid islands composed of ovoid cells with clear, PAS-negative cytoplasm. Atypical mitotic figures were also observed. Immunohistochemical analysis showed positivity for MUC-1, CK7, CK14, AE1/AE3, P53, and Ki-67. The final diagnosis was mucoepidermoid carcinoma with high-grade transformation. The patient was referred to an oncology hospital, where the proposed management included surgical removal followed by clinical and tomographic follow-up.
