Resumo: Canalicular adenoma (CA) is a benign epithelial neoplasm of the salivary glands, predominantly arising in minor salivary glands and typically presenting as a small, slow-growing, submucosal nodule in the upper lip of elderly individuals. We report an unusual case of CA in a 57-year-old male who presented with a painless, fibrous, normochromic swelling measuring 6 cm in diameter, with a duration of three years, located in the upper lip mucosa. The main clinical hypothesis was of pleomorphic adenoma, and an incisional biopsy was performed. Histopathological analysis revealed monomorphic epithelial ductal cells arranged in anastomosing cords within a richly vascular stroma, confirming the diagnosis of CA. Conservative surgical excision was carried out uneventfully, and no recurrence was observed during a one-year follow-up. This case highlights a rare presentation of CA with an unusually large size, which may pose a diagnostic challenge in the clinical evaluation of upper lip nodules.
