Resumo: A 55-year-old female patient, undergoing induction treatment with the HyperCVAD chemotherapy protocol for acute lymphoid leukemia (ALL), and with diabetes mellitus as a comorbidity, was admitted to a reference hospital due to a febrile neutropenia episode presenting an ulcerated lesion on the lower lip resulting from reactivation of the herpes simplex virus. Due to thrombocytopenia (50,000 platelets/mm³) and leukopenia (500 leukocytes/mm³), the lesion evolved with intense bleeding and crust formation. For local management, photodynamic therapy (PDT) was applied every 48 hours, intercalated with photobiomodulation therapy (PBMT) using red and infrared wavelengths simultaneously, along with lip hydration with vitamin E and enhanced oral hygiene care measures. As systemic therapy, acyclovir was instituted. Local therapies (PDT, PBMT) were essential for pain control and promotion of healing of viral lesions in the immunocompromised patient.
