Título: Architectural and Cytological Features of Oral Epithelial Dysplasia: Correlation with Malignant Transformation

Resumo: Oral epithelial dysplasia (OED) is a potentially pre-malignant disorder with a variable risk of progression to squamous cell carcinoma (SCC). Objective: To analyze the association between architectural and cytological features listed in the 5th edition of the WHO Classification of Head and Neck Tumours and the risk of malignant transformation (MT).Study Design: Clinical data and histological slides from 147 OED cases were retrieved. Slides were digitized and re-evaluated for 28 WHO-listed histological features. Chi-square or Fisher's exact tests were used to assess associations with MT. Results: Among the 147 cases, 85 (57%) were male and 62 (42%) female, with a mean age of 59.2 years (range: 18-90). Ten cases (6.8%) developed SCC. Fourteen features showed significant association with MT (p ≤ 0.05), including eleven architectural features (irregular stratification, lost of basal cell polarity, drop-shaped rete processes, basal cell clustering/nesting, mitoses high in the epithelium, mitoses in maturing cells, reduced keratinocyte cohesion, verrucous/papillary architecture, sharply defined lesion, multiple patterns of dyplasia, and multifocal or skip lesions) and three cytological features (increased mitotic activity, atypical mitotic figures, and apoptotic mitoses). Conclusion: Several WHO-listed histological features were significantly associated with malignant transformation in OED, supporting their prognostic relevance.
