Resumo: Introduction: Neoplasms that affect the facial region, especially on the lips, can represent a challenge in diagnosis. In this context, two benign neoplasms stand out: pleomorphic adenoma (PA) and chondroid syringoma (SC). Case report: male patient, 27 years old, complaining of asymptomatic nodular enlargement on the left upper lip. He underwent incisional biopsy, the anatomopathological diagnosis was compatible with low-grade malignant neoplasia of the minor salivary gland. Based on this diagnosis, surgical treatment was planned to resect the lesion and reconstruct the lip. Analysis of the entire surgical specimen revealed two new diagnostic possibilities, both mixed tumors: AP and SC. After an in-depth histopathological analysis, components of cutaneous appendages were excluded, resulting in the final diagnosis of AP. Conclusion: PA is a benign neoplasm of the salivary glands, rare in the upper lip. An adequate distinction from other skin tumors is requeride so that the correct treatment can be guided.
