Título: Systemic Inflammation and Clinical Outcomes in Oral Squamous Cell Carcinoma: A Survival-Based Analysis

Resumo: Objective: To evaluate the involvement of systemic inflammation in tumor development and its impact as a prognostic biomarker of clinical outcomes in patients with oral squamous cell carcinoma (OSCC). Study Design: A total of 71 patients with OSCC were recruited from oncological referral hospitals in Espírito Santo. Patients were classified according to clinical stage (I-II and III-IV) and were followed for up to 24 months or until death, whichever occurred first. Absolute counts of neutrophils, eosinophils, and lymphocytes were collected, along with the neutrophil-to-eosinophil ratio (NER) and neutrophil-to-lymphocyte ratio (NLR). Cutoff points were determined using ROC curve analysis. Overall survival (OS) was assessed using the Kaplan-Meier method and the log-rank test. Results: Elevated NER was related to worse OS in patients with advanced-stage disease (III-IV) (p < 0.05). NLR and the absolute counts of neutrophils, eosinophils, and lymphocytes showed no significant association with clinical stage or OS. Conclusion: Elevated NER reflects an inflammatory environment that promotes tumor progression and poor prognosis. It emerges as a simple and effective inflammatory marker for predicting overall survival in patients with advanced-stage OSCC.
