Resumo: A 13-year-old male patient presented to a hospital outpatient clinic with asymptomatic swelling in the right hemiface, evolving over approximately one year. Clinical examination revealed a firm mass in the right maxillary region, with no associated tooth mobility. Computed tomography showed a well-defined mass affecting the right maxilla and extending into the maxillary sinus. An incisional biopsy was performed, and histopathological analysis revealed a benign neoplasm composed of bony trabeculae and osteoid lamellae, densely lined by osteoblasts within a highly cellular fibroblastic stroma. The diagnosis of Juvenile Trabecular Ossifying Fibroma was established. Rapid lesion growth was noted after the first surgical intervention. A surgical resection was then performed, and histopathological examination confirmed the initial diagnosis. Postoperatively, the patient developed wound dehiscence and an oroantral communication, although the nasal mucosa remained intact. At the three-month follow-up, the patient showed no clinical or tomographic signs of recurrence and remains under outpatient monitoring.
