Título: MORPHEA INVOLVING THE LIPS AND GINGIVA: A DIAGNOSTIC CHALLENGE

Resumo: Morphea is a subtype of scleroderma, a rare autoimmune inflammatory condition characterized by excessive collagen deposition, typically confined to the skin. Cases with mucosal involvement are uncommon but clinically significant. A 23-year-old female presented with significant gingival recession in the upper right anterior region and a linear white patch on the gingiva, labial mucosa, and upper lip. Imaging revealed localized bone loss, periodontal ligament thickening, and generalized pulp stones. Biopsies of the gingiva and labial mucosa showed fibrous connective tissue with dense collagen deposition, as well as perivascular and perineural inflammatory infiltrates. The correlation between clinical findings, specific complementary exams, and rheumatologic assessment led to the diagnosis of localized scleroderma. The patient was treated with topical clobetasol propionate 0.05%, resulting in stabilization of the oral manifestations; however, she remains under rheumatologic follow-up. This case highlights the importance of differential diagnosis among autoimmune diseases with rare oral manifestations.
