Resumo: An 85-year-old male farmer with Parkinsons disease and palliative prostate cancer treatment developed a painful, bleeding mandibular tumor over six months. After tooth extraction for a presumed dental infection, the lesion doubled in size within three months. The patient had a 70-year history of heavy smoking and alcohol use, with 80% tobacco-related lung damage. Examination revealed cachexia, fixed submental and submandibular lymphadenopathy, and a 10 cm irregular, ulcerated mass involving the anterior mandible and floor of the mouth, with radiographic evidence of bone resorption. Initial suspicion of prostate metastasis was excluded by biopsy showing undifferentiated malignancy. CT scan demonstrated bilateral lymph node involvement without distant metastases. Immunohistochemistry confirmed Atypical Malignant Neural Granular Cell Tumor (S100+, NSE+, CD68+; negative for pan-cytokeratin, P40, 34BE12, SOX10, HMB-45, Melan-A, ALK1, and CD56). The patients death three months after diagnosis. This case highlights the importance of dental surgeon training for improving healthcare quality.
