Resumo: We report a case of a 40 years old female patient admitted in outpatient care with facial asymmetry due to mandibular swelling with an intraoral proliferative nodule with erythematous appearance and blackened areas. Due to its clinical aspect of malignancy, was requested emergency biopsy without preoperative exams. Was then performed an incisional biopsy, the lesion presented excessive bleeding. Microscopic sections revealed intense lymphocytic infiltrate consisting of atypical plasma cells with large, hyperchromatic nuclei, binucleated, atypical mitoses, lymphocytes with large, hyperchromatic, edentate nuclei and macrophages with giant nuclei and foamy cytoplasm arranged in angiocentric and angiodestructive patterns. Immunohistochemistry revealed MUM1 +, Bcl2 +, EBV +, PAX5 +, CD79a +, CD10 + and Ki-67 on +95% of cells, confirming diagnosis of high-grade b-cell lymphoma. HIV serology was requested, which came back positive, the tumor was later surgically removed. The patient started treatment with HAART and it's under 1 month post-surgery follow-up.
