Resumo: A 68-year-old male with a dermatologist-confirmed diagnosis of lichen planus presented with painful burning sensations and dry mouth. Intraoral examination revealed atrophic and erosive lesions with marked sensitivity. The patient was refractory to systemic corticosteroids and did not respond to topical clobetasol rinses. Systemic treatment was changed to methotrexate; however, during dose adjustment, symptoms persisted, and conventional analgesics failed to provide relief. As an adjunct approach, full-spectrum cannabidiol (CBD) oil (3000 mg/30 mL) was introduced at a dose of two drops daily in the morning. Within one week, the patient reported notable improvement in pain and burning. No adverse effects were observed. CBD therapy was maintained as part of the ongoing management plan, contributing to sustained symptom control. This case highlights the potential of cannabidiol oil as a promising alternative for managing pain and discomfort in patients with immune-mediated oral diseases, particularly when conventional therapies prove ineffective.
