Título: Gingival exophytic lesion as a manifestation of mandibular traumatic neuroma

Resumo: Traumatic neuroma is a reactive proliferative lesion resulting from nerve injury, often associated with dental surgical procedures. Healing complications, infection, and chronic irritation may contribute to its development. A 45-year-old woman with controlled diabetes and hypertension, presented a dental infection. Extraoral examination revealed right mild submandibular and cervical swelling, tender on palpation. Intraorally, a 2.5 cm exophytic, normochromic, lobulated, and firm lesion was observed between right mandibular molars, extending slightly to premolars, along with extensive caries on second mandibular molar. Imaging exams showed bone rarefaction in the mental region extending to incisors and molars, an impacted tooth mandibular canine, and a radiolucent apical area with furcation involvement on second mandibular molar. An incisional biopsy of the firm, non-bleeding lesion was performed, followed by extraction the second molar and curettage. Histopathology revealed a traumatic neuroma and a periapical granuloma, respectively. The patient returned after three months for surgical excision.
