Resumo: Intraosseous vascular malformations (IVM) represent <1% of intraosseous tumors and rarely affect jawbones. A 2-year-old male patient presented with a purplish intraoral swelling on the right posterior mandible. Computed tomography demonstrated an extensive multilocular expansive lesion involving the mandible body and ramus, with displacement of the germ of the first permanent lower molar and the second deciduous molar, the latter with resorption. An incisional biopsy and histopathological analysis were performed. Moreover, immunohistochemistry showed positive results for CD34, CD31, and D2-40. Based on findings, the diagnosis of mixed IVM with arterial, venous, and lymphatic components was established. The treatment consisted of embolization, using radioscopy, followed by surgery with mandible and permanent tooth preservation. Bone neoformation, without recurrence, was observed in 4 years. Despite its rarity, IVM requires accurate diagnosis. Furthermore, embolization reduced the risk of profuse bleeding and permitted surgery that preserved the integrity of bone structure.
