Resumo: We report a case admitted outpatient care with a facial swelling and pain in a 52 years old female patient. Clinically manifested as a tumoral mass in the vestibular mucosa of the left maxilla with a granulomatous appearance, sessile base and irregular, telangiectatic surface. Preoperative exams included coagulation tests, fasting blood glucose and blood count. The patient underwent biopsy surgery, during the procedure was performed a triple-site incisional biopsy. Microscopic sections revealed oral mucosa fragment with presence of atypical mitoses, hyperchromatic cells with loss of nucleocytoplasmic ratio, keratin pearls, islets and epithelial cords in perivascular and perineural invasion. Immunohistochemistry revealed P53 +, P16 and Ki-67 on 60 to 70% of cells, staging revealed cT4cN3bcM0, confirming diagnosis of moderately differentiated stage IVB squamous cell carcinoma. There was a delay in oncological treatment, hemimaxillectomy and bilateral neck dissection were performed. The patient passed away 6 months after the initial diagnosis.
