Resumo: A 39-year-old female patient with systemic lupus erythematosus was referred for dental care due to painful oral lesions that emerged after discontinuation of systemic corticosteroids. Clinical examination revealed ulcerated and erosive lesions on the buccal mucosa and lips, accompanied by white striae and non-removable plaques, consistent with lupus-related mucosal involvement. Periodontal disease was also present. Initial treatment included topical corticosteroids, photobiomodulation therapy (PBM), and supragingival scaling, with partial improvement. During treatment, the patient was hospitalized due to a genital cytomegalovirus infection, and concurrent worsening of oral lesions raised suspicion of cytomegalovirus coinfection in the oral cavity. Antimicrobial photodynamic therapy was administered empirically during this period. Following resolution of the systemic condition, intraoral PBM and intralesional corticosteroid injections were resumed, resulting in significant healing, alongside the reintroduction of systemic corticosteroids. This case underscores the need for interdisciplinary care and dental follow-up in managing oral lesions in immunosuppressed patients with autoimmune diseases.
