Título: ORAL CHRONIC GVHD IN ALLOHSCT PATIENTS: 12-YEAR RETROSPECTIVE ANALYSIS FROM A BRAZILIAN CANCER CENTER

Resumo: Objective:
To evaluate the prevalence, clinical characteristics, and progression of oral chronic graft-versus-host disease (cGVHD) in allogeneic hematopoietic stem cell transplantation (alloHSCT) recipients at a Brazilian cancer referral center over a 12-year period.
Study Design:
This retrospective cohort study analyzed 60 surviving patients who underwent alloHSCT between January 2012 and September 2024. Clinical and demographic data were retrieved from electronic and physical medical records. Oral cGVHD manifestations were assessed using National Institutes of Health (NIH) criteria. Statistical analyses were performed with SPSS version 29.0, with significance set at p ≤ 0.05.
Results:
The cohort consisted of 45.0% males and 55.0% females, with a mean age of 40.1 years. Acute myeloid leukemia (40.0%) and acute lymphoblastic leukemia (20.0%) were the most frequent indications for transplantation. Oral cGVHD was present in 40.0% of patients, with lichenoid lesions (84.0%), erythema (56.0%), pain (56.0%), ulcers (36.0%), and xerostomia (32.0%) being the most common symptoms. Symptoms began on average 198 days post-transplant and resolved around day 337. Myeloablative conditioning was significantly associated with oral cGVHD (p = 0.000).
Conclusion:
Oral cGVHD is a prevalent, prolonged complication post-alloHSCT, underscoring the need for early diagnosis, multidisciplinary care, and long-term follow-up, particularly in underserved populations
