Resumo: In approximately 60% of cases, the initial signs of pemphigus vulgaris appear in the oral mucosa, yet they are frequently overlooked in the absence of skin lesions. This report presents two cases of oral pemphigus vulgaris, illustrating the impact of early versus late diagnosis on outcomes. In the first case, delayed diagnosis led to extensive oral epithelial desquamation, deep blistering, bleeding, secondary infection, severe discomfort, 40 kg weight loss, and multiple hospitalizations. Misdiagnosis was attributed to insufficient awareness of oral manifestations and poorly located biopsies. In contrast, the second case involved a patient who presented with a burning sensation near tooth 13. Clinical examination revealed a flaccid epithelium and a positive Nikolsky sign elicited by an air jet. Prompt recognition led to early diagnosis and successful management with minimal discomfort. These cases emphasize the importance of early identification of oral lesions in improving prognosis and reducing morbidity in pemphigus vulgaris.
