Resumo: A 38-year-old woman presented with a 30-day history of left submandibular swelling (5 cm) and concurrent floor-of-mouth swelling (3 cm), both asymptomatic, smooth, soft, and without inflammatory signs. Radiography revealed no calcifications. Ultrasound identified a well-defined anechoic lesion (~7 cm³) with thick walls in the sublingual space, extending into the submandibular space via the mylohyoid muscle, consistent with plunging ranula. Biopsy confirmed mucous extravasation. Management included marsupialization followed by decompression with a Foley probe, performed 30 days apart. Complete regression occurred within 7 days. Six-month follow-up ultrasound showed a residual scar and transient seroma, which resolved spontaneously. No recurrence was observed after 10 months. This case highlights ultrasounds utility in diagnosing and monitoring the submandibular region by using a high-frequency linear probe, which provides images with higher spatial resolution and clearer visualization of salivary gland abnormalities.
