Título: Intraosseous Arteriovenous Malformation in a Pediatric Patient: Surgical Challenges and Interdisciplinary Approach

Resumo: An 11-year-old female presented with facial asymmetry and a firm swelling in the left mandible, progressing since age six. Initially suspected to be odontogenic myxoma, a surgical attempt was made but suspended due to severe bleeding, raising suspicion of an intraosseous arteriovenous malformation (AVM). An incisional biopsy confirmed intraosseous AVM. Computed Tomography and Magnetic Resonance Imaging revealed an expansile bone lesion with disorganized trabeculae and dilated vascular channels. Angiography showed a heterogeneous tumor blush arising from branches of the left external carotid artery. Selective embolization with polyvinyl alcohol particles and Gelfoam was followed by complete surgical excision, including peripheral ostectomy and multiple tooth extractions. Histopathological and immunohistochemical analysis confirmed the AVM. After one year of follow-up, facial symmetry and absence of recurrence were observed. This case emphasizes the importance of a multidisciplinary approach and the role of preoperative embolization in managing high-flow vascular lesions of the mandible.
