Resumo: Mucocutaneous leishmaniasis primarily affects the skin and mucous membranes, typically manifesting as ulcers or nodules. Lip involvement is uncommon, especially in younger individuals. A 17-year-old female with no underlying medical conditions presented with a two-month history of a painful, swollen lower lip lesion accompanied by white plaques and lymphadenopathy. Initial treatment with antivirals, antibiotics, and a 15-day course of prednisone yielded minimal improvement. Serological tests for herpes, chlamydia, leishmaniasis, and paracoccidioidomycosis returned negative results. The patient was subsequently referred to the Department of Oral and Maxillofacial Surgery for further evaluation, where an incisional biopsy was performed. Histopathological examination revealed intense epithelial hyperplasia, atypical reactive cells, chronic inflammation, and intracellular oval corpuscles consistent with leishmaniasis. Treatment with Glucantime® was initiated, but due to the development of pyoderma in the affected area, adjunct therapy with ceftriaxone, metronidazole, and fluconazole was required. Following this combined regimen, the ulcer and nodule showed significant improvement.
