Resumo: A male patient, aged 43, with a 28-year history of smoking and no other known comorbidities, presented with a white plaque located in the floor of the oral cavity, measuring 3 cm, homogeneous, with soft consistency, and asymptomatic. The lesion had been evolving for one year. Given the clinical suspicion of leukoplakia, which is an oral potentially malignant disorder, the patient was counseled on the associated risks and the importance of controlling predisposing factors, which led to smoking cessation. Incisional biopsy revealed hyperkeratosis without epithelial dysplasia. Subsequently, an excisional biopsy of the lesion was performed using a high-intensity laser device. The patient has remained free of clinical recurrence after one year of follow-up. This case underscores the importance of early diagnosis, appropriate management, and effective control of risk factors-particularly tobacco exposure. Evidence-based clinical decision-making and patient adherence are essential for preventing malignant transformation and promoting oral health.
