Título: METASTATIC HEPATOCARCINOMA PRESENTING AS A MANDIBLE FRACTURE

Resumo: A 63-year-old male was referred to an emergency service complaining of acute intense pain in the left posterior mandible. Clinical examination revealed a swelling in the area covered by normal skin and normal oral mucosa. Panoramic radiograph revealed an ill-defined radiolucency associated to a mandibular fracture in the region. Medical history revealed controlled systemic arterial hypertension. Surgical exploration of the area for reduction of the mandibular fracture revealed the presence of a friable hemorrhagic soft tissue proliferation. Histological analysis of the tissue revealed a proliferation of large atypical and pleomorphic cells with abundant eosinophilic cytoplasm disposed in sheets and sinusoid spaces. Immunohistochemical reactions positive for HepPar and TTF-1, and negative for CK7, CK20, PAX8 and GATA3 rendered a diagnosis compatible with metastatic hepatocarcinoma. The patient was referred to gastrointestinal evaluation and was diagnosed with an advanced hepatocarcinoma. He died from the disease a few months later.
