Título: Diffuse Large B-Cell Non-Hodgkin Lymphoma Involving the Maxilla

Resumo: A 62-year-old female patient was referred from primary care with a progressive maxillary lesion persisting for over four months, accompanied by fetid odor, impaired speech, and oro-nasal communication. Clinical examination revealed a vegetative lesion with erosive areas and bone destruction. An initial biopsy of the nasal cavity floor was inconclusive. A subsequent incisional biopsy of the anterior maxillary lesion, analyzed at the Oral Histopathology Laboratory of Piracicaba Dental School, demonstrated a solid proliferation of large lymphoid cells exhibiting marked pleomorphism and numerous atypical mitoses. Immunohistochemical analysis showed positivity for LCA, CD20, PAX5, CD10, BCL6, and C-MYC, and negativity for CD3, BCL2, and MUM1. The Ki-67 proliferation index was approximately 90%. These clinical and histopathological findings supported a diagnosis of diffuse large B-cell lymphoma. The patient was referred to the hematology-oncology department at the Federal Districts base hospital and is currently undergoing treatment and follow-up.
