Resumo: Maxillary osteosarcoma is a rare malignant bone tumor in children. This report describes an uncommon case involving an 11-year-old patient who presented with a rapidly expanding lesion in the anterior maxilla, associated with swelling and tooth mobility. Histopathological analysis revealed an atypical mesenchymal neoplasm with chondro-osteoblastic differentiation. The diagnosis of osteogenic osteosarcoma was confirmed through immunohistochemistry and radiographic imaging. The patient underwent complete surgical resection with tumor-free margins, followed by adjuvant chemotherapy. Functional and aesthetic rehabilitation was achieved with a partial removable prosthesis. The patient remains disease-free after 10 years of consistent oncologic and dental follow-up. This case highlights the importance of early diagnosis and a multidisciplinary approach in the successful management of pediatric maxillary osteosarcoma.
