Título: EBV CO-INFECTION ASSOCIATED WITH SEVERE MUCOSAL MANIFESTATION OF PEMPHIGUS VULGARIS

Resumo: The importance of viral co-infection in the severity of pemphigus vulgaris (PV) has been suggested. The aim of this case report is to describe an extensive PV associated with EBV infection. A 65-year-old man was admitted with ulcerated and painful lesions in the oral mucosa, dysphagia, dysphonia, and weight loss. The oral mucosa showed multiple ulcerated and bleeding lesions, with positive Nikolsky sign. Videolaryngoscopy and endoscopy confirmed the extensive involvement of the larynx and esophagus. Histology of the oral mucosa showed suprabasal acantholytic mucositis. Immunofluorescence was positive for anti-IgG in epidermal keratinocytes surface. Both oral and esophagus biopsy demonstrated cytopathic changes suggestive of Herpesviridae. PCR (oral mucosa and blood) were positive for EBV. Treatment included intravenous acyclovir, corticosteroids, cyclophosphamide and rituximab. Patient showed good clinical response, reinforcing the need for an integrated approach in extensive and atypical presentations such as viral coinfections, which significantly influence prognosis and therapeutic decision-making.
