Resumo: Reactive infectious mucocutaneous eruption (RIME) has a distinct pathophysiology, milder course, and better prognosis when compared to Stevens-Johnson syndrome/toxic epidermal necrolysis. We report a case of a 16-year-old female patient who presented to the emergency room with fever and extensive oral lesions. She was diagnosed with influenza B two weeks prior, with improvement after antiviral treatment (oseltamivir). One week later, she developed lip vesicular lesions and productive cough, being treated initially with corticoids and then with acyclovir and antibiotics after pneumonia and secondary herpetic infection were suspected. Oral examination revealed erosions on the tongue, palate, buccal mucosa and floor of the mouth, and hemorrhagic crusting of the lips. Skin or ocular lesions were absent. Serological tests were negative to HSV1/2, CMV, EBV and toxoplasmosis, and positive to Mycoplasma pneumoniae, confirming the diagnosis of RIME. Patient was treated with corticoids, antibiotics and photobiomodulation, with resolution of oral lesions after two weeks.
