Título: APICAL PERIODONTITIS AND MEDICATION-RELATED OSTEONECROSIS OF THE JAWS (MRONJ) - A SYSTEMATIC REVIEW

Resumo: Objective: This pioneering systematic review aims to investigate the role of apical periodontitis (AP) in the development of Medication-related osteonecrosis of the jaws (MRONJ).
Study design: This systematic review was reported following the Preferred Reporting Items for Systematic Review and Meta-analysis (PRISMA). The protocol was registered on the International Prospective Register of Systematic Reviews (PROSPERO). Independent reviewers performed a systematic search in PubMed/MEDLINE, Web of Science, Scopus, Embase, and ProQuest. Eligibility criteria consisted of: (1) in vivo studies investigating MRONJ in AP regions; (2) Use of antiresorptives, or other medications with MRONJ potential. Studies with a diagnosis of other intraosseous lesions were excluded.
Results: From 239 studies, 11 were included. The most used drug was Zoledronic Acid, in doses ranging from 66-300 µg/kg, for 3 to 12 weeks. Induction of periapical lesions was performed mainly before the administration of the antiresorptives. Clinical or histological evidence of MRONJ in areas of AP was reported in 72%. Areas with AP presented more frequently with MRONJ, mucosal healing defects and nonviable osteocytes.
Conclusions: AP seems to play a role as a risk factor for MRONJ, emphasizing the importance of early diagnosis and management of endodontic infections in patients undergoing antiresorptives.
