Resumo: A 42-year-old male with B-cell Non-Hodgkin Lymphoma in remission since 2021 presented in 2024 with myalgia and cutaneous lesions after traveling. He initially had vesicles on the face, trunk, and limbs, which evolved into crusted ulcers with pruritus and fever. Arboviruses, Monkeypox, and varicella-zoster were considered. Serologies were negative for dengue (NS1), syphilis, HIV, and hepatitis B, but positive for herpes simplex types I and II. Acyclovir was started, but symptoms persisted, including myalgia and severe cramps. During hospitalization, the Hospital Dentistry team identified oral lesions on the lip and tongue were noted, and further testing was requested. Histology confirmed cutaneous leishmaniasis, revealing moderate inflammatory infiltrate with macrophages and ovoid, eosinophilic structures compatible with amastigote forms of Leishmania sp. The patient began a ten-day course of liposomal amphotericin B. Despite treatment, his condition worsened, requiring a second hospitalization, after which he unfortunately passed away.
