Resumo: Severe cutaneous adverse reactions are potentially fatal manifestations characterized by delayed-type hypersensitivity. Between them, the DRESS syndrome (Drug Reaction with Eosinophilia and Systemic Symptoms) stands out, frequently associated with anticonvulsants. This study reports the case of a 72-year-old black female diagnosed with a brain tumor using Phenytoin and Dexamethasone. After four weeks of using these medications, she presented fever, odynophagia, dyspnea, and skin rashes, progressing to respiratory failure. During hospitalization, she developed painful ulcers on the gums, lip, and lower lip mucosa, with a scaly and friable appearance. A biopsy revealed extensive epithelial necrosis, spongiosis, exocytosis, and intense inflammatory infiltrate mixed with intravascular eosinophilia. Laboratory tests indicated eosinophilia and elevated liver transaminases, contributing, together with the clinical history, to a diagnostic hypothesis compatible with DRESS. Systematic treatment was carried out with corticosteroid therapy and antihistamines. For the oral lesions, laser therapy (3J) was used daily, resulting in complete remission.
