Resumo: Mucocutaneous leishmaniasis (MCL) is a severe infection that can be difficult to diagnose. The objective is to describe a case of MCL with exclusive oral lesions in an immunosuppressed patient. An 82-year-old woman was referred for evaluation of painful oral lesions. Granulomatous, bleeding lesions with ulcerated and necrotic areas involving the anterior maxilla were observed. The patient had myelodysplastic syndrome and a history of untreated self-limited cutaneous leishmaniasis 5 years ago. The initial biopsy showed lymphoplasmacytic infiltrate with granulomas, but no leishmania (L). PCR, immunohistochemistry, and all serologies and cultures were negative. A second biopsy maintained the same pattern. However, oval structures positive for anti-leishmania antibody were observed. A third biopsy showed positive PCR for leishmania. Lesions showed significant reduction after liposomal amphotericin B, and patient has been monitored. This case highlights the importance of medical history and the challenge of diagnosing MCL in the granulomatous phase in immunosuppressed patients.
