Resumo: Amelogenesis imperfecta is associated with the risk of caries. The objective of this case is to report the potential association between amelogenesis imperfecta and recurrent periapical abscesses. A 7-year-old girl with recurrent infections of dental origin. She presented a syndromic facial profile, multiple gingival fistulas, teeth with malformation of enamel, periapical lesions (radiographic images), and no caries. The infectious condition only stabilized after extraction of all deciduous teeth. The same infections occurred related to permanent teeth, requiring hospitalization for antibiotic therapy, with resolution only after tooth extraction. No functional and/or structural immunological alterations were identified, and the test for rickets was negative. The rare genome collection identified mutation in the FGFR2 and FAM20C genes. The histological analysis confirmed enamel malformation with increased dentin permeability. This case suggests that the permeability of the dental structure can enable the access of microorganisms to the supporting tissues, facilitating the occurrence of recurrent infections.
