Resumo: A plunging ranula is a mucus extravasation phenomenon, in which mucin dissects through the mylohyoid muscle, causing cervical swelling, with or without involvement of the floor of the mouth. This paper reports a case of a 29-year-old male patient, with a history of unsuccessful marsupialization of a ranula larger than 5 cm. Extraoral examination showed a right submental/submandibular swelling; intraorally, a lesion occupied the entire floor of the mouth. Imaging exams revealed homogeneous thin-walled cystic lesions in the sublingual and submandibular spaces. The largest lesion, located on the right side (6.8 x 2.6 cm), displaced the submandibular gland laterally and compressed muscular structures. Its medial component extended between the geniohyoid/genioglossus and mylohyoid muscles. Histopathology showed mucus extravasation with intense inflammation and foamy macrophages, confirming the diagnosis of plunging ranula. Treatment involved enucleation of the ranula from the sublingual and submandibular regions and removal of the right sublingual gland.
