Título: AGGRESSIVE CRANIOFACIAL OSTEOSARCOMA WITH PALATAL AND MAXILLARY INVOLVEMENT IN A YOUNG ADULT: A THERAPEUTIC CHALLENGE

Resumo: A 33-year-old woman, passive smoker, presented with a painful palatal nodule without ulceration, associated with left nasal obstruction for five months. Oroscopy revealed a violaceous, multinodular fibrotic lesion affecting the left hard and soft palate, without cervical lymphadenopathy. CT scan showed a hypodense expansile mass in the left maxillary sinus, extending to the choanal region, palate, and paranasal walls. MRI confirmed a heterogeneous lesion with bone erosion in the maxilla, nasal septum, ethmoid sinuses, and left orbital floor. Endoscopic resection revealed a poorly differentiated malignant neoplasm with myxochondroid matrix, suggestive of osteosarcoma, and positive margins. Neoadjuvant chemotherapy with cisplatin and doxorubicin was initiated. Following multidisciplinary discussion, radical surgery was performed: total maxillectomy, left hemimandibulectomy, and regional flap reconstruction. The 12.5 × 9.4 × 8.9 cm specimen confirmed predominantly myxoid osteosarcoma with chondroblastic component. Immunohistochemistry: CD99+, S100−, Ki-67: 25-35%. Adjuvant chemotherapy was started. Patient remains under multidisciplinary follow-up.
