Resumo: A 54-year-old white female patient was referred to our service for evaluation of painful ulcerated lesions on the oral mucosa. Extraoral examination revealed vesiculobullous lesions on the hands, arms, and neck. Intraoral examination identified multiple ulcerations affecting the bilateral buccal mucosa, tongue, and anterior mandibular gingiva. Based on clinical presentation, autoimmune vesiculobullous disorders were considered in the differential diagnosis. Serological tests were requested, and an incisional biopsy of the perilesional area was performed. While serological results were negative, direct immunofluorescence demonstrated linear IgG deposits along the basement membrane zone. The correlation between the clinical features and histopathological findings confirmed the diagnosis of bullous pemphigoid. The patient was referred to rheumatologist for specialized treatment and follow-up. This report highlights the importance of the dental professionals in recognizing and diagnosing mucocutaneous manifestations of autoimmune bullous disorders.
