Resumo: Ameloblastic carcinoma is a rare neoplasm, primarily due to its low incidence. It is most frequently observed in individuals from the third decade of life onward, although cases have been reported across a wide spectrum of ages. The carcinoma typically manifests approximately a decade following the management of the primary tumor. A 40-year-old female patient was referred for evaluation after a radiographic finding in the mandible. Imaging revealed a poorly defined radiolucent osteolytic lesion in the mandibular body, involving the premolars, with a "floating teeth" appearance. Clinically, a painless swelling was noted, without ulceration, present for approximately two months. An incisional biopsy showed odontogenic epithelium with cellular pleomorphism and hyperchromatic nuclei, leading to the diagnosis of ameloblastic carcinoma. The patient underwent segmental mandibulectomy with cervical neck dissection. Follow-up over 28 months showed no recurrence or metastasis, and the patient remains disease-free.
