Resumo: Central Giant Cell Granuloma (CGCG) of the jaws is a benign osteolytic lesion with sometimes aggressive behavior. This case report describes an 18-year-old male with no relevant medical history, presenting with a painless swelling in the left paranasal region. CT imaging revealed a hypodense lesion of approximately 18 mm between teeth 22 and 23, causing root displacement. An incisional biopsy showed multinucleated osteoclast-like giant cells in a vascularized fibrous stroma. Normal parathyroid hormone levels ruled out systemic causes, confirming CGCG. Conservative treatment with 25 intralesional corticosteroid injections reduced the lesion to 8 mm, after which curettage was performed. The patient remains under follow-up with satisfactory bone regeneration. This case supports the use of intralesional corticosteroid therapy as a conservative approach for CGCG, potentially reducing surgical morbidity and improving outcomes.
