Resumo: A 26-year-old brown female patient with tuberous sclerosis and sought care from the oral and maxillofacial surgery department after noticing a nodule on the gingiva of the lower left anterior teeth, approximately 4 mm in size. During the anamnesis, she reported taking everolimus, undergoing neurological treatment, and mentioned two previous episodes of recurrence of the intraoral lesion. Cone-beam computed tomography revealed a well-defined hypodense image, with slight buccal cortical expansion and preservation of the bone margins. An incisional biopsy followed by curettage was performed. The initial diagnostic hypothesis was a hamartoma related to tuberous sclerosis, however, histopathological analysis confirmed the diagnosis of cemento-ossifying fibroma, a condition rarely identified in patients with tuberous sclerosis. Subsequently, the upper right third molar was extracted due to painful symptoms. The patient has been under clinical and radiographic follow-up for six months, showing normal healing and no signs of recurrence.
