Resumo: Infected odontogenic cysts are uncommom but can occasionally result in abscess formation. When odontogenic keratocysts become infected, they may manifest as pain, trismus, purulent exudates and fever. We present the case of a 24-year-old male who was initially evaluated by the Head and Neck Surgery team for a mandibular lesion characterized by a persistent abscess, unresponsive to multiple interventions. Both imaging and clinical assessments suggested a potential neoplastic process. However, histopathological examination confirmed the diagnosis of an odontogenic keratocyst. The patient was subsequently referred to the Oral and Maxillofacial Surgery Department, where the lesion was successfully enucleated along with the extraction of the associated tooth. The postoperative recovery was uneventful, and no recurrence of infection was observed over a 3-year follow-up period. This case highlights the importance of considering alternative sources of abscess beyond periapical and periodontal causes. The prognosis for infected cysts is favorable following complete surgical removal.
