Resumo: Herpetic stomatitis is an acute viral infection, usually caused by the Herpes simplex virus, characterized by painful ulcerations of the oral mucosa. This paper reports the case of a 53-year-old female patient who presented with multiple symptomatic, red ulcers with target-like appearance and ill-defined borders, located on the lower labial mucosa, attached gingiva, hard palate, buccal mucosa, labial commissure, ventral and tip of the tongue, along with similar lesions in the genital region. Differential diagnoses included Crohns disease, pyostomatitis vegetans, leishmaniasis, and autoimmune diseases. Crohn's disease was ruled out, as genital involvement is not reported in the literature. The patient had pancytopenia; serological tests for HIV and syphilis were negative. Incisional biopsy revealed ulceration with fibrinopurulent membrane, acantholysis, ballooning degeneration, and multinucleation of keratinocytes, accompanied by a mixed inflammatory infiltrate. The diagnosis was consistent with herpetic stomatitis, with recommendation for correlation with serological tests and investigation of possible immunodeficiency.
