Resumo: Intraductal carcinoma arising from a minor salivary gland at the base of the tongue is a rare and atypical occurrence. This case report describes an 86-year-old female patient with fair skin who presented to the Araújo Jorge Cancer Hospital with an endophytic, infiltrative lesion measuring 2 cm. On palpation, cervical lymph nodes were suspicious for malignancy, and a CT scan revealed a hypodense lesion with poorly defined borders in the right oropharyngeal region. An incisional biopsy was performed, and histopathological analysis showed cystic areas lined by apocrine cells exhibiting marked pleomorphism, arranged in papillary and micropapillary patterns, along with inflammatory infiltrate and areas of necrosis. Immunohistochemical evaluation was conducted using AE1/AE3, EMA, CK7, CK14, and CK19. The patient underwent partial glossectomy with right-sided cervical lymph node dissection. The surgical specimen confirmed the previous diagnosis, and the patient remains under follow-up, with no evidence of recurrence to date.
