Resumo: Peripheral odontogenic fibroma is a rare mesenchymal neoplasm. A female patient presented with a well-defined and painless nodular lesion located on the lingual surface of the gingiva between the right mandibular canine and lateral incisor, of unknown duration. The lesion showed, pedunculated base, smooth surface, and coloration similar to the oral mucosa. Imaging examination revealed superficial bone resorption in the same region. Under the clinical hypothesis of non-neoplastic proliferative process, the lesion was excised. Histopathological analysis revealed densely fibrous connective tissue with rich cellularity and sparse nests and strands of odontogenic epithelium dispersed within the stroma. The odontogenic epithelium was positive for CK19. SMA displayed focal positivity in the walls of blood vessels and muscle. HHF-35, CD34, and h-caldesmon were negative. The final diagnosis was peripheral odontogenic fibroma. The patient is under clinical follow up and no signs of recurrence were observed after 8 months of treatment.
