Resumo: A 38-year-old female patient presented to the oral diagnosis clinic at São Leopoldo Mandic School of Dentistry complaining of a nodule between teeth 41 and 42, with progressive growth over the past five months. Clinical examination revealed an exophytic, firm nodular lesion with a smooth to slightly lobulated surface, erythematous coloration, and superficial ulceration, measuring approximately 1 cm. The adjacent mucosa showed signs of inflammation. Initial diagnostic hypotheses included pyogenic granuloma and inflammatory fibrous hyperplasia. An excisional biopsy was performed using a high-power diode laser, and the specimen was submitted for histopathological analysis. Microscopic examination showed cellular connective tissue with areas of ossification and cementoid material, confirming the diagnosis of peripheral ossifying fibroma. This case highlights the importance of clinical reasoning when facing reactive lesions with nonspecific features, reinforcing the essential role of histopathology in diagnostic accuracy and the effectiveness of diode laser as a conservative surgical approach.
