Título: Management of oral lesions in a patient with toxic epidermal necrolysis: a case report

Resumo: A 43-year-old woman was admitted to a referral hospital with an erythematous maculopapular rash and desquamation on the face, trunk, and neck; genital ulcerations; and ocular mucosal dryness, following a history of systemic antibiotic and anti-inflammatory use for dysuria treatment. Oral examination revealed limited mouth opening, ulcerated labial commissures, crusted lips, diffuse ulcerations on the tongue and palate, erosions on the buccal mucosa and dry mouth sensation associated with mucosal dryness. Based on the clinical presentation, the diagnosis was consistent with Toxic Epidermal Necrolysis. Systemic treatment included cefepime and dipyrone. Oral mucosal management involved 0.12% chlorhexidine, antimicrobial Photodynamic Therapy (aPDT) on the tongue and commissures, red photobiomodulation therapy (PBMT) on the buccal mucosa, desonide® for lips and PBMT protocol for hyposalivation. Oral lesions began to regress after seven days, allowing the patient to transition from a liquid to a pasty diet. Pacient remains under outpatient follow-up without signs of complication.
