Resumo: Sialolipoma is a rare histological variant of lipoma, characterized by a well-circumscribed proliferation of mature adipose tissue associated with salivary gland elements. Its main differential diagnosis is lipomatosis, a non-neoplastic condition typically associated with systemic disorders such as diabetes mellitus, liver cirrhosis, chronic alcoholism, and malnutrition. We report the case of a 60-year-old male presenting with an asymptomatic nodule on the lower lip. The patient did not report any of the systemic conditions commonly linked to lipomatosis. Histopathological examination of an incisional biopsy revealed mature adipose tissue interspersed with normal salivary gland parenchyma, consistent with sialolipoma. Accurate diagnosis is essential to distinguish this entity from other lipomatous lesions of the salivary glands and to ensure appropriate conservative surgical management.
