Resumo: A 61-year-old woman, former smoker/alcohol user, with a history of hypertension, rheumatoid arthritis, and sinusitis, was referred for evaluation of an oral condition. Upon physical examination, a papule on the left buccal mucosa, close to the upper second molar was observed. It was normochromic with a yellowish center, smooth surface, sessile base, hard consistency, and measured 3 mm. Sialolithiasis in the parotid papilla was the clinical hypothesis. Saliva was not observed upon manual expression of the gland, and a periapical radiograph of the affected area showed a circular radiopacity, confirming the diagnosis. The patient was instructed to stimulate salivary flow, but the lesion persisted. Sialolith excision was performed with suturing to prevent the stone from returning to the duct. She is doing well and has no complaints. The sialolith could be removed without complications due to its superficial location at the parotid duct opening, with rapid resolution of the condition.
