Resumo: The buccal mucosa (BM) is a common site of squamous cell carcinoma (SCC) in Southeast Asia due to tobacco-chewing habits but is rare in Western populations. An 82-year-old normosystemic male, chronic alcohol consumer with no tobacco history, presented with oral soreness and discomfort. Intraoral exam revealed an endophytic, asymptomatic lesion with indurated borders on the right BM. Incisional biopsy confirmed moderately differentiated SCC with muscle invasion, keratin pearls, atypical mitoses, and contact with dysplastic surface epithelium. The lesion rapidly progressed into a symptomatic erythroleukoplastic nodule measuring 4 cm. The patient was referred for oncologic treatment. This case highlights the aggressive behavior of SCC in a non-traditional site and patient profile, emphasizing the potential role of chronic alcohol consumption-independently-in the pathogenesis and progression of oral SCC. It underscores the need to consider alternative risk factors and molecular or environmental influences, even in the absence of tobacco use.
