Resumo: Acute herpetic gingivostomatitis (AHGS) is often the first manifestation of HSV-1, presenting with painful oral lesions, fever, and feeding difficulties. Case 1: A 4-year-old boy had fever, nasal congestion, and feeding refusal. Initially suspected of otitis media, oral exam revealed ulcerative lesions and gingival edema. He was treated for AHGS with IV acyclovir and photobiomodulation, recovering in five days. Case 2: A 2-year-old girl presented with fever, cough, and feeding refusal, unresponsive to amoxicillin. Oral findings included tongue lesions, gingival hyperplasia, and perioral crusting. She received IV acyclovir, oral care, and photobiomodulation, and was discharged after seven days. Case 3: A 12-year-old boy had fever and feeding refusal while on amoxicillin. Exam showed coalescent ulcerative lesions and diffuse gingival edema. Management included IV antivirals, photobiomodulation, and supportive care, with full recovery in nine days. Oral Medicine specialists play a key role in diagnosing and managing AHGS in pediatric inpatients.
